Derived this driver from the ModemSetup-2 Sample-Code which comes with the Newton Developer CD.

-> Creatix LC 144 VF Data Fax Voice (from http://www.creatix.de) !

Works with Fax/Data connects over the 'serial' external port, supports FaxClass1 & 2. Use on NOS 2.x only ! It was only tested on a MP2100 via the Interconnect port, and worked well.
One ceveat:
In Germany only, the redial lock feature could not be disabled via the AT%TCB command :( !
So redials are only made after 30 Sek. since the last unsuccessful dial attempt ! 
This modem uses a Rockwell chipset !

P.S.: I was unable so far to establish a fax-poll call with my MP 2100 and this modem, no matter what settings I chose, Newton wouldn't want to collect :(!
I believe this a bug in NewtonOS !
